Ebola is the common term for a group of viruses belonging to genus Ebolavirus, family Filoviridae, which cause Ebola hemorrhagic fever.[1] The disease can be deadly and encompasses a range of symptoms including vomiting, diarrhea, changes in skin color, general body pain, internal and external bleeding, and fever.[2] Mortality rates are generally high, ranging from 50% - 90%,[3] with the cause of death usually due to hypovolemic shock or multiple organ failure.[4] The virus is named after the Ebola River in the African nation-state of the Democratic Republic of the Congo (formerly Zaïre), near the site of the first outbreaks.[5] The Democratic Republic of Congo has been the site of four recent outbreaks, including one in May 2005. Ebola is believed to be a zoonotic virus, although despite considerable effort by the World Health Organization, no animal reservoir capable of sustaining the virus between outbreaks has been identified. One possible candidate reservoir is the fruit bat.[6] Because Ebola is lethal and since no approved vaccine or treatment is available, Ebola is classified as a Biosafety Level 4 agent, as well as a Category A Bioterrorism agent[7] and a select agent by the CDC. The symptoms of Ebola are rather similar
to that of the Marburg virus, which is also in the family Filoviridae.

Ebola hemorrhagic fever is one of the deadliest of a group of diseases called viral hemorrhagic fevers. They range in seriousness from relatively mild illnesses to severe and potentially fatal diseases. All forms of viral hemorrhagic fever begin with fever and muscle aches. Depending on the virus, the disease can get worse until the patient becomes very ill with breathing problems, severe bleeding (hemorrhage), kidney problems, and shock. Viral hemorrhagic fevers are caused by viruses from four families: filoviruses, arenaviruses, flaviviruses, and bunyaviruses. The usual hosts for most of these viruses are rodents or arthropods (such as ticks and mosquitoes). In some cases, the natural host for the virus is not known.
What is the infectious agent that causes Ebola hemorrhagic fever? Ebola hemorrhagic fever is caused by several Ebola viruses. Ebola viruses are members of the filovirus family; when magnified several thousand times by an electron microscope, these viruses look like threads (filaments). Ebola virus was discovered in 1976 and named for a river in Zaire, Africa, where it was first detected.
Where is Ebola hemorrhagic fever found? Ebola viruses are found in Central Africa. The source of the viruses in nature remains unknown. Monkeys, like humans, appear to be susceptible to infection and might serve as a source of virus if infected.
How do people get Ebola hemorrhagic fever? People get the disease by direct contact with virus-infected blood, body fluids, organs, or semen. The disease is spread mainly by close person-to-person contact with severely ill patients. This happens most often to hospital-care workers and family members who care for an ill person infected with Ebola virus. Close personal contact with persons who are infected but show no signs of active disease is very unlikely to result in infection. Transmission of the virus has also been linked to the re-use of hypodermic needles in the treatment of patients. Re-using needles is a common practice in developing countries, such as Zaire and Sudan, where the health-care system is underfinanced. Medical facilities in the United States do not re-use needles. Ebola virus can be spread from person to person through sexual contact. Persons who have recuperated from an illness caused by Ebola virus can still have the virus in their genital secretions for a short time after recovery and can spread the virus through sexual activity.
What are the signs and symptoms of Ebola hemorrhagic fever? People infected with Ebola virus have sudden fever, weakness, muscle pain, headache, and sore throat, followed by vomiting, diarrhea, rash, limited kidney and liver functions, and both internal and external bleeding. Death rates range from 50% to 90%.
How soon after exposure do symptoms appear? Symptoms begin 2 to 21 days after infection.
How is Ebola hemorrhagic fever diagnosed? Diagnosis requires specialized laboratory tests on blood specimens. Handling blood from a persons infected with Ebola virus is an extreme biohazard and can be done only in specially equipped laboratories. Diagnosis in patients who have died can be made by testing tissue samples.

How common is Ebola hemorrhagic fever? Until recently, only three outbreaks of Ebola hemorrhagic fever in humans had been reported. The first two, in 1976 in Zaire and in western Sudan, were large outbreaks that resulted in more than 550 cases and 340 deaths. The third outbreak, in 1979 in Sudan, was smaller, with 34 cases and 22 deaths. In each of these outbreaks, most cases occurred in hospitals where medical supplies were inadequate and where needles and syringes were re-used. The outbreaks were quickly controlled by isolating sick patients in a place requiring the wearing of mask, gown, and gloves; sterilizing needles and syringes; and disposing of wastes and corpses in a sanitary way. In 1995, an outbreak in Kikwit and surrounding areas in Bandundu Province, Zaire, caused 316 deaths. The outbreak was amplified in a hospital by staff who became infected through poor nursing techniques. At the request of health officials in Zaire, medical teams from CDC and the World Health Organization, and from Belgium, France, and South Africa, collaborated to investigate and control the outbreak. Two isolated cases of Ebola hemorrhagic fever were identified in Cote d'Ivoire in 1994-1995. The most recent outbreaks were in rural Gabon in 1994 and in 1996. A patient from the 1996 Gabon outbreak traveled to Johannesburg, South Africa, and fatally infected a health-worker there as well.
How can Ebola hemorrhagic fever be prevented? Under normal circumstances, travelers are at low risk of getting the disease. To eliminate the risk, travelers should avoid areas where Ebola outbreaks are occurring.

Many of the stories about Ebola in Preston's book The Hot Zone are refuted in the book Level 4: Virus Hunters of the CDC by Joseph B. McCormick, an employee of the CDC at the time of the early outbreaks. One pervasive myth follows that the virus kills so fast that it has little time to spread. Victims die very soon after contact with the virus. In reality, the incubation time is usually about a week. The average time from onset of early symptoms to death varies in the range 3-21 days, with a mean of 10.1. Although this would prevent the transmission of the virus to many people, it is still enough time for some people to catch the disease. Another myth states that the symptoms of the virus are horrifying beyond belief. Victims of Ebola suffer from squirting blood, liquefying flesh, zombie-like faces and dramatic projectile bloody vomiting, at times, from even recently deceased. In actual fact, only a fraction of Ebola victims have severe bleeding that would be even somewhat dramatic to witness. Approximately 10% of patients suffer some bleeding, but this is often internal or subtle, such as bleeding from the gums. Ebola symptoms are usually limited to extreme exhaustion, vomiting, diarrhea, abdominal pain, a high fever, headaches and other body pains. The following is an excerpt from an interview with Philippe Calain, M.D. Chief Epidemiologist, CDC Special Pathogens Branch, Kikwit 1996: At the end of the disease the patient does not look, from the outside, as horrible as you can read in some books. They are not melting. They are not full of blood. They're in shock, muscular shock. They are not unconscious, but you would say 'obtunded', dull, quiet, very tired. Very few were hemorrhaging. Hemorrhage is not the main symptom. Less than half of the patients had some kind of hemorrhage. But the ones that had bled, died. Ebola Test Urgent Amid Globalism The very deadly Ebola Virus. "Global travel and global trade means that everything goes everywhere," W. Ian Lipkin, director of the Greene Infectious Disease Laboratory at Mailman, told United Press International. "SARS proved that. It started in China and moved to Toronto. While most of these diseases are endemic to Africa, a case of Ebola virus was diagnosed five years ago in Reston, VA after a shipment of animals arrived from the Philippines," he said. Lipkin noted that the new test developed by the Columbia researchers marks an important breakthrough in battling the cadre of exotic-sounding, but increasingly well-traveled maladies. "Diagnosis and isolation are the key to stopping an outbreak. Up until now there were no good tests for any of these problems and the disease had to become full blown before it could be recognized," he said. "Now we can test sick people who have recently traveled in an area where a disease is endemic, or handled animals from that area, and make a diagnosis before they have a chance to spread the virus," Lipkin said. According to a MSPH, the research team that devised the new test was led by Thomas Briese and represented an international coalition of laboratories from the United States, Canada, Europe, South Africa, India and China, the same group that first worked together on the SARS outbreak in 2003. The group decided to stay together and tackle other global health problems, and the test for hemorrhagic fevers is the result. Diagnostic tests for diarrheal diseases and meningitis/encephalitis are being developed, and the respiratory virus test developed for SARS and other viral respiratory pathogens is also in use. The new test that emerged from the collaboration takes two hours to simultaneously diagnose a virtual plethora of deadly pathogens, including yellow fever, Ebola Zaire virus, Ebola Sudan virus, Marburg virus, Lassa virus, Rift Valley fever, Crimean-Congo hemorrhagic fever, Hantaan virus, Seoul virus and Kyansanur Forest disease. A combination of technologies, including polymerase chain reaction, genetic amplification and mass spectroscopy, are involved, but the cost of the test is only $5 to $10. Lipkin told UPI that all the hemorrhagic diseases covered by the test are fatal, quickly become epidemic in humans and their early symptoms all resemble mild flu. He noted that the animal connection was an important one because 70 percent of all infectious diseases (such as HIV, Lyme disease and West Nile virus) begin in animals and jump to humans, including the viruses that the new test covers. Rona Hirschberg, senior program officer at NIAID, told UPI that the technology, if available decades ago, could have changed the course of history in the battle with another infamous filler. "If we had known about HIV in chimps back in the seventies, we would have had diagnostic tests and possible treatments ready and could have slowed the epidemic," Lipkin responded. "The diseases our new test covers have already moved to humans, but being able to diagnose them quickly will allow us to stop them before they can devastate the world's population the way HIV has done."
